Internal Moderation (IM) Form and Cover Sheet
An Internal Moderation (IM) Form Internal Moderation Form Cover Sheet must be submitted to UFA whenever:
- New candidates need to be registered with OCN.
- Learners have evidence for one of more Units which they wish to submit for accreditation.
The Internal Moderation Form Cover Sheet asks you to provide:
 | contact information for your project |
| the name of the internal moderator |
| invoicing details (if different from those of the internal moderator) |
| an indication as to whether the claim is for an external moderation event (EME) or is via certification claim status (CCS) |
| if relevant, the date of the EME you wish to attend |
| details of any equipment which you will need at the EME (for example a CD or DVD player). |
The Internal Moderation Form allows up to 8 learners to be registered and/or claim Units for accreditation. Additional sheets may be submitted as required.
When completing the form please follow the guidance example form closely and bear in mind:

Legibility – Please use capital letters and print or type learners’ details as clearly as possible. Mistakes made at this stage prove costly and time consuming to correct later. The information you provide will be used to register the learner with OCN for life and will be used to produce certificates. It is therefore extremely important that names are spelt correctly, that correct dates of birth are provided and that handwriting is legible. Unclear forms may be returned. |

New Registration? – If this is the learner’s first unit of accreditation write “yes” in the space. |

Assessor/IM details – Please list the assessor and internal moderator for each unit. |
|
Internal moderation – Please complete his section as fully as possible. |
Wherever possible IM Cover Sheets and IM Forms should be sent to UFA at least 6 weeks prior to external moderation events.
